When to submit a network connection form:
- New computer needs to go on the network.
- Computer on the network is moving to a new location.
- different desk
- different room in same building
- different building
- Computer is being replaced with a new computer.
- Computer is going to be retired.

Note: Once submitted, the request cannot be acted on until a signature from the PI is received.
