Tax Sheltered Annuity (403(b)) Plan


Tax Sheltered Annuity Plan is a voluntary savings program that allows employees to make pre-tax contributions to various mutual funds and insurance company annuities. Additionally, Cash Balance Retirement plan participants who contribute to the Tax-Sheltered Annuity (TSA) plan will receive a match of up to 50% on the first 6% of their TSA contributions. The match will be in the form of pay credits added to the TSA plan..

Forms

Fidelity Investments Beneficiary Designation Form - This form is used to change your beneficiary designation.

Fidelity Transfer/Rollover Form - This form is used to transfer a retirement plan (i.e., 401(k), 403(b), 401(a), 457(b)) from a previous employer into the TSRI 403(b) plan or from a current TSRI 403(b) vendor into Fidelity Investments. If you have not already established an account, other forms may be needed.

Fidelity Investments Distribution Form - This form is used to complete a distribution from your account
